Title	Explanation
First Name / Given Name	Your First Name / Given Name. It is important that you provide your name in the correct order.
Last Name / Family Name	Your Last Name. If you do not have a last name on your passport, then enter all your first name in the last name field as well.
File Number	The 9 digit file number beginning with a letter. You will find this number, also referred to as an Application Number, on any official document issued to you from the visa office where you submitted your application. Your Immigration File Number/Application Number starts with a letter and is followed by a combination of nine (9) numbers or letters. (Example: B-000-000-000 or E-P00-000-000) E = Immigration (GCMS) B = Immigration (CAIPS) F = Immigration (Family Class) S = Student Visa V = Temporary Resident Visa W = Temporary Worker Visa
Date of Birth	Your date of birth. It should be in the following order: Year-Month-Date.
Primary E-mail	Primary contact e-mail address.
Alternate E-mail	Required only if we are unable to reach you at the Primary e-mail.
Additional Notes	Any special instructions or notes you would like to mention.
